Oncology Clients:  If you are in active treatment for cancer (presently receiving radiation or chemotherapy), or between treatments, or within 6 months of your last treatment, you will need to print the Physician's Approval Form above and have your oncologist approve your massage therapy treatment, in addition to completing the Oncology Intake Form above.
